Criminal Reference Check Form and LinksUpdated Tuesday May 21, 2019 by Orillia and District Soccer.
Criminal Reference Check is required for all coaches and referees within the Orillia and District Soccer Club. The Orillia and District Soccer Club pays the cost of the process as we deeply appreciate the time all coaches provide for the youth in our club.
All travel and competitive coaches must have this completed annually. House League coaches only have to complete the process every three years provided they coach consecutively during that time.
Please complete the attached form and take a copy of the letter from the Club to the local police detachment. The letter will waive the $25 fee. You will require two pieces of ID; one must be photo (Health Card can not be one of them); preferably Driver's Licence and Passport. Detachment hours are 8:15am-5:00pm and is located on Peter Street. Residents of Oro-Medonte will need to go to the Barrie OPP detachment.
Please note that Criminal Reference Checks take 4-6 weeks to be returned so time is of the essence. You will be notified by the OPP or local police department when it is complete and ready for pick up.
